For DeVry or Keller Graduate School of Management alumni:

Graduates please download and complete the transcript request form (PDF) and send it to the appropriate fax number or mailing address of the last location you attended. The form provides a drop down selection list in the "Last Location Attended" field from which you can select the appropriate location and populate the form with the correct address, fax, and phone number for the location selected. Please submit the form to the location selected.
